Client forms (pre-appointment)

Menu Appointments → Client forms (when the module / feature is enabled for the store).

Prepare consent or intake forms that clients complete before (or around) an appointment — via a link sent by email or SMS.

What it is for

Need Use
Consent Have clients acknowledge information before a treatment
Intake Collect history, allergies, useful details for staff
Bilingual French and English content required before activation / client send
Linked to services Attach a form to services or sub-services

Create or import a form

  1. Open Client forms.
  2. Create a form in the designer (text, checkboxes, signature, etc.), or
  3. Import a PDF and choose:
    • FR or EN only — fills that locale; Also complete the other language (on by default) can auto-translate structure / labels;
    • FR + EN — one bilingual PDF (often half / half) fills both locales at once.
  4. Both text PDFs and scans are supported (server-side OCR). Circles become radio choices, squares become checkboxes — review in the designer.
  5. Check / complete both languages (name + content) before activating.
  6. Link the relevant services / sub-services and choose the send frequency.

On edit, Import a PDF (language) can fill FR or EN without recreating the form (same mirror-translation option).

Send frequency (with appointment reminders)

Option Meaning
First clinic visit Once per client in this store
First linked sub-service First time a linked sub-service is on the appointment
Every linked appointment Every appointment that includes a linked sub-service (default)

Automatic send follows appointment email/SMS reminders (when the module is ON and the form is active).

Send a test

On the list or edit page: Send a test.

  • Start from an appointment (pre-fills the client) or enter a recipient.
  • Works even if the form is not active yet.
  • Blocked if FR or EN is incomplete (same rule as client send).

Client side

The client opens the public link: header with their details (first name, last name, phone, email, date of birth), store-themed form, signature / fields as designed, storefront footer. Header fields are prefilled — no need to rebuild them in the designer.

Success criteria

  • I create or import a FR + EN form and activate it.
  • I link it to a sub-service and understand send frequency.
  • I send a test and open the link as a client would.
  • An appointment reminder can trigger a send when frequency rules allow it.
